Evolution Healthcare collects, uses and stores health and personal information of patients and kiritaki[1] and personal information of its kaimahi[2], credentialled specialists, contractors and visitors for the purpose of carrying out its core functions and activities.
Our commitment
Evolution Healthcare is committed to safeguarding the privacy of personal and health information we hold. We have a legal obligation to comply with the requirements of the Privacy Act 2020 and the Health Information Privacy Code 2020. Evolution Healthcare Group requires all employees, contractors, volunteers, visiting health professionals and students to comply with New Zealand Privacy Legislation, and associated standards and codes.
Our purpose
Evolution Healthcare’s purpose is to enhance the health, recovery and wellbeing of New Zealanders. To achieve this, we need to collect, use and share information about people receiving our services.
What personal and health information we collect
We aim to collect only what is necessary for our lawful purposes. Personal information we may include:
- Contact Information – your name, date of birth, NHI (if available), address, email address, phone numbers, next of kin/emergency information.
- Demographic information – gender, nationality, ethnicity, country of residence
- Unique identifiers – ACC claim numbers, NHI numbers, employee numbers
- Financial information – bank account details
- Health Information – medical history required for the delivery of safe and effective healthcare delivery.
How we collect personal information
Whenever it is practical to do so, we will collect your information directly from you, unless you authorise the collection from another party. This may take place when you complete initial referral, admission or administration paperwork via the referral process, from a funder, via your general practitioner via email, or over the telephone.
Sometimes we may need to collect information from third parties such as a whānau member, power of attorney or another health service provider if it is unreasonable or impractical to collect it from you and with your authorisation to do so.
How we use information
We collect personal and health information from people who use our services to deliver appropriate medical care, rehabilitation and advice.
We use this information to:
- provide the required treatment, rehabilitation, service and advice
- administer and manage those services including charging, billing and debt collection
- contact you to provide advice, appointment details or information relating to your treatment or rehabilitation
- conduct appropriate health insurance/funder eligibility checks
- monitor the quality of our services.
- Meet our contractual obligations
Sharing and disclosure
We may share your information with other healthcare professionals and agencies involved in your care, rehabilitation and treatment. We only share your information for the purpose it was collected, with your authorisation or is legally required. This may include:
- your nominated General Practitioner
- the health care professional who referred you
- other healthcare professionals involved in your ongoing care.
- With third party service partners (ACC, MSD) and in relation to carrying out the services required
- Any government agency or regulator required by law
Mailing lists
When you register for one of your services, you may be added to our mailing list for relevant service updates and information. You may unsubscribe anytime using the links at the bottom of the update.
You may also be contacted to provide feedback regarding your experience an service delivery. This information is used to support quality improvement across the services we deliver.
Accessing your information
You have the right to request access to personal information we hold about you (including medical records) and to request corrections. Please submit requests in writing to the service provider where you received treatment.
We will require you to provide proof of identification. There is no charge for access to your medical records.
In limited circumstances where we may decline access, (such as where giving access would put your health or the privacy or safety of another person at risk); we will explain the reason in writing.

How to request correction of your personal information
If your personal information is incorrect, you may request a correction by emailing us.
We will respond, check our records and if the information is incorrect we will
- Update the information on your file
- Send the corrected information to any third party who may have received the information
- Notify you that we have made the change
Retention and disposal of information
We retain personal and health information only for as long as it is necessary for our lawful purposes and to meet applicable legal, regulatory, and clinical obligations.
Patient records are retained for no less than 10 years after the most recent date which an individual was provided services.
Privacy breaches and notifications
If we experience a privacy breach that has caused or is likely to cause serious harm, we will notify the Office of the Privacy Commissioner and affected individuals as soon as practicable, consistent with the Privacy Act. We will also take steps to contain, assess and remediate the breach.
